Who can Apply?

The Permanent License holder who's License is Torn / Defaced or Lost should apply for Duplicate License.

Documents Required

Form LLD
Old License if Torn / Defaced or Police Report if Lost
Two copies of recent passport size photographs
Fees Rs. 214

Note : No person shall hold more than one Driving License.
